We elevate the recovery of our clients by sealing the gaps & providing continuity of care after hospital or outpatient discharge. We provide exceptional care to all our clients by experienced nurses.
About Our Founder
Our founder, Antonette Carabuena, BSN, RN, PCCN, CDP is a dedicated registered nurse with over 14 years of experience in various medical settings.
She developed a broad medical background through hands-on nursing experience and training in several specialty areas, including Critical Care, Surgical, Telemetry, Orthopedics, Rehabilitation, Geriatric, Concierge and Private Nursing.
Antonette is a member of the American Association of Critical Care Nurses (AACN), Massachusetts Nurses Association (MNA), Hospice and Palliative Care Association (HPNA). She holds current certifications in Progressive Care Certified Nurse (PCCN), Acute Cardiac Life Support (ACLS) Basic Life Support (BLS) and Certified Dementia Practitioner (CDP).
